Please note that if you have insurance, choosing one of these self-pay packages waives your right to send the charges associated with the appointment to insurance.
By opting for the self-pay medical package, you acknowledge and agree that the cost of the services included in the package is your responsibility, and you will not seek reimbursement from any insurance provider. Additionally, the services rendered under this self-pay arrangement are not subject to insurance contractual rates, and no claim will be filed on your behalf with any insurance company.
